High quality equipment cannot ordinarily be made from existing equipment. They are not upgrades to existing equipment, but rather, replacements. A standard, steel or iron longsword may be a +0, or normal, longsword. A longsword using special materials or crafting techniques, such as an elven longsword, may be +1. A longsword whose blade is made from mithril may be +2 quality, and so on. The higher quality materials and craftsmanship used to make the equipment, the higher quality the resulting equipment is.

High quality weapons grant additional Accuracy and Damage equal to the quality level of the weapon. Therefore, a +1 weapon would grant a +1 bonus to attack rolls and +1 to the damage of attacks made with the weapon.
A higher quality set of armor grants additional AC equal to the quality level of the armor. Therefore, a +1 piece of armor would give 1 additional AC.
High quality catalysts grant additional Spell Accuracy and Damage with spell attacks made with the catalyst equal to the quality level of the catalyst. Therefore, a +1 Catalyst would grant a +1 bonus to Magic Attack rolls and +1 additional damage to spell attacks made with the catalyst.
High quality shields grant additional Defence equal to the quality level of the shield. Therefore, a +1 shield would grant +1 Defence against all damage types.
